Integrated Care System Development

Supporting the development of integrated care systems to deliver improved outcomes.

Integrated Care System

The challenge

Demand upon health and care services is growing - people are living longer, with more and complex health conditions. The challenge of meeting this demand has been amplified by constrained funding and a legislative framework that has not kept pace with changes on the ground or the needs of local populations.

The NHS Long Term Plan sets out how the NHS will move to a new service model in which patients have more options, better support and properly joined-up care at the right time in the optimal care setting.

Health and care leaders are grappling with the challenges of shifting from organisation-centric to place-based models of care, whilst at the same time trying to maintain or improve current levels of delivery performance. The development of Integrated Care Systems nationally is central to the delivery of the Long Term Plan.

Traditional strategies that have served leaders well, which focus on a top-down approach, often experience difficulties within a complex adaptive system and do not yield optimum results. False starts and vested interests are common.

Notwithstanding this, successful integration will undoubtedly enable a more resilient and sustainable health and care system that delivers improved outcomes for its population.

Our solution

Our team provides practical advice and support to help you navigate the landscape and work through, rather than around, problems. With extensive knowledge and experience, our people provide the support you need when you need it.

Integrated Care System DevelopmentOur solutions are as unique as you are, with complete flexibility to provide a 'one stop shop' for integrated care development.

Multiple building blocks require development to achieve an integrated care system, such as the right workforce, outcomes and culture. We review your existing services to help develop your vision and establish how best they should be formed around the person, neighbourhood, place and whole system.

Once the vision is established, we help to design and mobilise it, underpinned by a programme management office solution to enable the vision to become reality.

We understand the needs of both commissioners and providers, and support whole-system transformation enabling meaningful integration resulting in improved outcomes.
