The challenge

Timely, efficient and safe discharge from acute care is a high priority for every health and social care economy. For the acute services, effective discharge processes support the flow of non-elective admissions from the Emergency Department and capacity for elective activity by reducing the levels of stranded and super stranded patients.

Evidence also indicates that discharge processes which promote timely and safe transfer out of acute settings can save costs to the whole health system as more patients maintain their level of independence, reducing reliance upon extended health and social care services (NHS England, 2014).
